HERA welcomes applicants for the Center for Disease Control's Community Transformation Grants (CTGs). The CTGs will support the implementation, evaluation, and dissemination of evidence-based community preventive health activities to reduce chronic disease rates, prevent the development of secondary conditions, address health disparities, and develop a stronger evidence base for effective prevention programming. HERA is poised to help in every step of the process, from grant writing and preparation to implementation.
